Inpatient care is medical treatment administered to a patient whose condition requires treatment in a hospital or other health care facility, and the patient is formally admitted to the facility by a doctor.

A variety of imaging techniques such as X-ray radiography, ultrasound, computed tomography (CT), nuclear medicine including positron emission tomography (PET), fluoroscopy, and magnetic resonance imaging (MRI) are used to diagnose or treat diseases. Interventional radiology is the performance of usually minimally invasive medical procedures with the guidance of imaging technologies such as those mentioned above.

Bone densitometry

Bone densitometry, also called dual-energy x-ray absorptiometry, DEXA or DXA, uses a very small dose of ionizing radiation to produce pictures of the inside of the body (usually the lower (or lumbar) spine and hips) to measure bone loss. It is commonly used to diagnose osteoporosis, to assess an individual's risk for developing osteoporotic fractures. DXA is simple, quick and noninvasive. It's also the most commonly used and the most standard method for diagnosing osteoporosis.

This exam requires little to no special preparation. Tell your doctor and the technologist if there is a possibility you are pregnant or if you recently had a barium exam or received an injection of contrast material for a CT or radioisotope scan. Leave jewelry at home and wear loose, comfortable clothing. You may be asked to wear a gown. You should not take calcium supplements for at least 24 hours before your exam.

Breast Imaging

Breast imaging is a test that uses a tracer and special camera to find breast cancer. Rather than simply taking a picture of a breast, molecular breast imaging is a type of functional imaging. This means that the pictures it creates show differences in the activity of the tissue.

CT Scan

A computerized tomography (CT) scan combines a series of X-ray images taken from different angles around your body and uses computer processing to create cross-sectional images (slices) of the bones, blood vessels and soft tissues inside your body. CT scan images provide more-detailed information than plain X-rays do.

Echocardiogram

An echocardiogram (echo) is a graphic outline of the heart's movement. During an echo test, ultrasound (high-frequency sound waves) from a hand-held wand placed on your chest provides pictures of the heart's valves and chambers and helps the sonographer evaluate the pumping action of the heart.

MRI

Magnetic Resonance Imaging (MRI) is a non-invasive imaging technology that produces three-dimensional detailed anatomical images. It is often used for disease detection, diagnosis, and treatment monitoring. It is based on sophisticated technology that excites and detects the change in the direction of the rotational axis of protons found in the water that makes up living tissues.

Clinical Medical laboratories are an example of applied science, as opposed to research laboratories that focus on basic science, such as found in some academic institutions.

Emergency medicine is not defined by location but may be practiced in a variety of settings including, but not limited to, hospital-based and freestanding emergency departments (EDs), urgent care clinics, observation medicine units, emergency medical response vehicles, at disaster sites, or via telehealth.

Emergency medicine encompasses planning, oversight, and medical direction for community emergency medical response, medical control, and disaster preparedness. Williamson Memorial is committed to treating all patients regardless of their ability to pay and providing financial assistance to persons who have healthcare needs and are uninsured, underinsured, ineligible for a government program, or otherwise unable to pay for their medical care based on their individual financial situation under our hospital’s Financial Assistance Program. No patient will be denied financial assistance due to his or her race, religion, national origin or any other basis prohibited by law.

Financial relief may be available to patients who receive non-elective care, do not qualify for state or federal assistance, and are unable to pay a substantial portion of their balance, either directly or through a third-party payer source.

We offer a Charity Care Discount under our Financial Assistance Program to patients who qualify under our policy, based on multiples of the U.S. Federal Poverty Guidelines, which will be evaluated using certain financial criteria and supporting documentation to verify income provided by the patient. A patient may apply for the Charity Care Discount by filling out our Financial Assistance Program (“FAP”) Application and returning it to the hospital, along with any required documentation, at the time of service or any time after care is provided during the patient’s billing cycle.

Patients who do not qualify for our Charity Care Discount may still be eligible for financial relief under the hospital’s Financial Assistance Program. Patients without insurance or any other third-party payer source, who are not eligible for government assistance or our hospital’s Charity Care Discount, will receive a discount from the hospital’s billed charges, referred to as the “Uninsured Discount.” Additionally, a patient may also be eligible for a “Catastrophic Care Discount,” in the event the patient’s account exceeds a certain percentage of the patient’s gross annual income.

We also provide eligibility screening services that can help identify the availability of resources to cover medical services, such as Medicaid.

Billing and Insurance

When you receive a bill from Williamson Memorial, it covers services you received at one of our healthcare facilities. As a courtesy to our patients, we will bill your insurance company, Medicare or Medicaid on your behalf. We will do everything possible to obtain payment and expedite your claim.

You also may receive separate bills from your personal physician, surgeon, pathologist, radiologist or other healthcare professional. Questions about any of these bills should be directed to the number printed on the statement you receive.

If you have insurance:

We accept most major health insurance plans and managed care programs.

Several factors can affect the amount a patient owes the hospital. Some health insurance plans may have special requirements for certain tests or procedures. Additionally, some physicians may not participate in your healthcare plan and their services may not be covered. If you fail to meet your health insurance plan requirements, you may be financially responsible for all or part of the services you receive at the hospital. Be sure you review your benefits plan to understand all of the plan requirements affecting your financial responsibility.

If you don’t have insurance:

No one will be denied necessary medical care due to lack of insurance or inability to pay. However, you may be asked to pay a deposit when you’re admitted or when you register for an outpatient procedure.

Payment Policy

Payment in full is required at the time of service from all patients who owe co-pays, coinsurance, have not met their deductible, or do not have insurance coverage. Balances after insurance payment are due upon receipt.

Even if we bill your health insurance, payment is the ultimate responsibility of the patient. You will be financially responsible for payment of your hospital bill. To assist you with your financial responsibility, we offer discounts, payment plans or charity care, for patients that qualify. Williamson Memorial strives to provide comprehensive, quality healthcare in a spirit of personal caring, safety and concern. We believe that you are entitled to specific rights and you may have the responsibility to make decisions regarding your healthcare.

You have the right to considerate and respectful care.

You have the right to obtain from your physician complete, current information concerning your diagnosis and prognosis in terms that you can reasonably understand.  When it’s not medical advisable to give such information to you, it should be made available to an appropriate person on your behalf.  You have the right to know the name of the physician responsible for coordinating your care.

You have the right to expect that all communications and records pertaining to your care will be treated confidentially.

You have the right to expect that, within its capacity, Williamson Memorial must make a reasonable response to your request for services.  The hospital must provide evaluation, service and referral as indicated by the urgency of your case. The institution to which you are being transferred must first have accepted the transfer.

You have the right to obtain information about any relationship of your hospital to other health care institutions insofar as your care is concerned.  You have the right to obtain information about the existence of professional relationships among individuals, by name, who are treating you.

You have the right to be advised if the hospital proposes to engage in or perform human experimentation affecting care or treatment. You have the right to refuse to participate in such research projects. 

You have the right to expect reasonable continuity of care. You have the right to know in advance what appointment times physicians are available and where.  You have the right to expect that the hospital will provide a way for your physician or a representative of your physician to inform you about continuing health care requirement discharged from the hospital.

You have the right to examine and receive an explanation of your bills regardless of the source of payment.

At Williamson Memorial, our goal is to improve the quality of our patients’ care by providing information to patients and families so they can make informed choices and their wishes respected.

Advance Directives or “Living Wills” are legal documents outlining a patient’s general wishes for medical care. These are completed in advance" of becoming too sick to make decisions for yourself. Advance directives are recommended for everyone age 18 and older (including mature and emancipated minors).

Whether you’re preparing for a hospital stay, or visiting a patient receiving care or to visit staff providing care, knowing where to go and what to bring will help make your visit to Williamson Memorial less stressful. We wish to provide you with helpful information you may need to plan your visit.

Emergency Department

● Two visitors will be allowed with each patient.
● During emergent situations/activities all visitors will remain in the waiting room.
● In special circumstances the nursing staff can modify practice to meet the needs of the patient and family.
● Patient/family will be asked to identify one contact person who will receive all information. This contact person will relay information to the rest of the family or support system.
● Visitors are to wait in the treatment room or waiting room only. Visitors are not allowed to wait in the hallways.

General Medical Floor

Open visiting hours are from 8 a.m. to 8 p.m.

● Visiting may be restricted by request of the healthcare team, should visitation interfere
with the patient's progress.
● 2 visitors per patient will be allowed at one time, when a roommate is present. Special
arrangements may be possible relocating to single rooms or using waiting rooms for
large gatherings.
● Children under the age of 16 must be accompanied by an adult and stay with the adult when
visiting a patient.
● In order to assure patient privacy and the prompt delivery of care, visitors may be asked
to wait in the waiting room.
● Large families are encouraged to designate one or two contact person(s). This person
would then receive all status updates.
● Pediatric patients under the age of 18 must have one parent/guardian remain with them
around the clock.
● Any visitor staying overnight will sleep in the family lounge, on rollaway or recliner in the
patient room. At no time is the visitor to sleep in the second bed if one is in the room.
Visiting hours from 8 p.m. to 8 a.m. are only allowed when a special circumstance has occurred
and can be approved by the nurse providing care to the patient. Examples include but not
limited to:

● Critical and end of life changes in patient's condition
● Special needs of patients including safety considerations and developmental needs.
● Appointments for family to meet with the physician providing care
● Infectious diseases can delay a patient's recovery. Active surveillance by the nursing staff is required to ensure visitors are free from obvious signs of contagion such as cough, cold, or open wounds.

Exceptions to Listed Policy:

● We reserve the right to temporarily amend visiting hours and guidelines due to community threat or illness.
Visitors may be restricted by expressed request of the patient or by the physician, nursing supervisor or nurse in charge of the unit when visiting may interfere with patient care or unit activities.
● Physician order
● Visitation may be restricted in instances of outbreaks such as influenza or Covid-19. This will be at the discretion of the Infection Control Committee/Committee Chair or by order of the Bureau of Public Health.
● Visitors with obvious respiratory illness or other communicable disease will be asked not to visit until symptoms subside.
● Visitation policy will be modified if family/visitors are exhibiting signs/symptoms of communicable disease or have known exposure.

FOR THE SAFETY AND CONSIDERATION OF ALL PATIENTS AND OUR TEAM MEMBERS,
VISITORS ARE TO ADHERE TO THE FOLLOWING GUIDELINES:

1. Visitors may be subject to screening for symptoms of acute respiratory illness before
entering the hospital.
○ Visitors should not come to the hospital if they have a cold, sore throat, fever,
cough or other transmissible illness.
○ All visitors are required to self-screen for COVID-19 upon entry to the hospital
2. Visitors are expected to follow verbal and/or posted instructions; for example, “No
Visitors” and/or “Isolation” signs.
3. Patients under “Isolation/Precautions” may have visitation arranged. The clinician,
patient and support person (visitor) will work collaboratively to determine a schedule.
Two visitors per day will be permitted. Extension of visitation and special consideration
beyond this will be on a case-by-case basis. Nursing will assist with training on required
personal protective equipment (PPE) prior to the visitor entering the patient’s room.
Visitors must wear PPE during the entire visit and stay in the patient’s room or they may
be asked to leave.
○ Visitors of patients under “Isolation/Precautions” will be screened when entering
the building and are not to go to other areas of the hospital including cafeteria,
gift shop, or other patient rooms. They should limit contact with other in the
elevators and lobby areas.
○ Visitors for Isolation patients should be 16 years of age or older to allow proper fit of PPE.
4. If a patient is requesting a visitor/family to stay overnight, efforts will be made to place the patient in a private room if available.
5. Ask visitors to leave the patient’s room during physician and/or provider examination unless the patient has requested the visitor to remain.
○ Visitors are to utilize waiting rooms or waiting areas when patients are in the operating room, testing areas, or receiving treatment.
6. To ensure a quiet environment, instruct to use cellular phones at a minimum and keep the “ringing” volume set to low or vibrate.
7. Direct visitors to use designated public bathrooms outside of the patient’s room.
8. Williamson Memorial does not allow firearms, alcohol, or illegal drugs on the premises. Security can only take possession for safe keeping of those items belonging to patients. Contact security if such items are suspected or found in the possession of a visitor.
○ The hospital and its owned property does reserve the right to search bags, boxes and other containers if the staff believes there is a reasonable suspicion of an item that may cause harm to the patient, staff or facilities.
9. Smoking/vaping is not permitted inside the hospital. Designated off campus smoking areas are available.

Any issues or concerns with visitors shall be referred to the unit director or nurse supervisor.

We will:

● Inform patients and/or their support person* of the right to receive or refuse visitors;
● Not restrict or limit visiting privileges on the basis of race, color, national origin, religion, sex, gender identity, sexual orientation, or disability;
● Limit number of visitors to two per patient at a time. Length and frequency of visitation depends on the condition of the patient and the level of treatment required.

Visitors agree to:

● Be respectful to staff, patients, other visitors and facility property
● Adhere to hospital policies and procedures which includes but is not limited to:
○ No running, horse play, yelling, cursing
○ No video or photography without expressed permission
○ No weapons or other contraband (see #12 above)

After hours visitation:

Visitors are to obtain an identification sticker from security after hours (8PM to 8AM) located in the emergency room.

When a visitor presents after hours, security will do the following:

1. Explain to the visitor that normal visiting hours are from 8AM to 8PM.
2. If the visitor cannot come back or has extenuating circumstances, (i.e. patient in critical condition, out of state, etc.) have the visitor complete the Visitation Book.
3. Security will gather identification from the visitor and place a phone call to the nursing unit where the patient is located.
4. Security will advise the floor of a potential after hours visitor and will ask for nurse approval.
5. If the patient’s nurse approves the afterhours visit, security will gather and review proper identification from the visitor and issue a visitor pass. Visitor passes are good for the day/or the overnight they are issued
6. If the patient’s nurse does not approve the afterhours visit, security will advise the visitor accordingly and suggest that they return during normal visiting hours.
7. At times, a visitor may want to question the decision made not to allow them access to the building. Those incidents should be reported to the nursing supervisor.

The nursing supervisor has final authority on visitation if it is observed that it is not contributing to the welfare and in the best interest of the patient.

*support person is defined as family member, friend or other individual who supports the patient during his or her hospital stay and may exercise the patient’s visitation rights on his or her behalf.

Parking

ADA parking is designated at the emergency department and front entrance. You may call (304) 235- 2500, the main hospital number prior to your visit if you need valet or any assistance entering the hospital.

Visitor parking is located in front of the hospital. Please use the Main Entrance for non-emergency visits during the hours of 8 a.m. to 8 p.m.

How can someone call me?

The telephone in your room cannot be dialed directly from outside the hospital. All calls must come through the hospital switchboard, (NEED NUMBER) Your family may call you at any time on most nursing units. We suggest that you designate one person close to you for family members and friends to call for information. Our staff is unable to screen calls or to provide information on your condition over the telephone in most cases. Your caller can ask the operator for you by name unless you have chosen "no information status."

Are cell phones allowed in the hospital?

Yes.

Can a family member spend the night with me?

Adequate rest is important for you. However, arrangements can be made for a family member to spend the night with you and many rooms include pull-out couches.

When will I have tests?

Tests and procedures are scheduled daily, with emergencies handled first. Your nurse will know if your test or procedure will be done in the morning or afternoon.

Where can I smoke?

Williamson Memorial Hospital is a smoke-free environment. Patients are not allowed to smoke. Help is available to stop smoking. Please ask your nurse for assistance. Visitors may smoke only in their cars.

What if I have questions about my bill?

I. USING AND RELEASING PROTECTED HEALTH INFORMATION

A. Without Your Written Permission. We have the right to use and share your health information for the

following reasons:

1. Treatment: Information obtained by a nurse, physician, or other member of our health care team, recorded in your medical record, may be used to help decide your future care. We may also share information to others providing you care. This will help them stay informed about your care.

2. Payment: We request payment from your health insurance plan. Health plans need information from us about your medical care. Information shared with health plans may include your diagnoses, procedures performed, or future recommended care.

3. Health Care Operations: We may use and share PHI for our health care operations, such as quality improvement activities, training programs, accreditation, certification, licensing or credentialing activities. For example, we may use PHI to review our treatment and services and to evaluate the performance of our staff.

4. Required or Permitted by Law: We may share PHI when we are required or permitted to do so by law. For example, we may release PHI to proper authorities if we believe that you are a possible victim of abuse, neglect, or domestic violence. We may also share PHI necessary to stop a serious threat to the health or safety of you or others. Other releases could include: public health activities; requests from state or federal agencies; law enforcement; court order or other lawful process; approved research; workers’ compensation claims; military or national security agencies, coroners, medical examiners, and correctional institutions.

B. Without Your Permission, And You May Object.

1. Fundraising: We may use PHI to contact you in an effort to raise money for our operations. We may also release PHI to a foundation that is related to us so that the foundation may contact you in an effort to raise money for its operations. Any fundraising communications with you will include a description of how you may opt out of receiving any further fundraising communications

2. Family and Other Persons Involved in Your Care. Unless you object, we may share your PHI with a family member, relative, close friend, or any other person you identify is involved in your medical care. We may share information to notify the person of your location general condition or payment related to your care.

3. Disaster Relief Efforts. We may share your protected PHI to a public or private entity authorized by law or its charter to assist in disaster relief efforts for coordinating notification of family members of your location, general condition, or death.

C. Needs Your Written Permission

1. Psychotherapy Notes. We must get your permission to use or release psychotherapy notes, unless the psychotherapy notes are:

(1) By the creator of the psychotherapy notes for treatment purposes,

(2) For our own training programs in which mental health students, trainees or practitioners learn to improve their counseling skills,

(3) To defend ourselves in a legal proceeding initiated by you,

(4) To a health oversight agency for oversight of the creator of the psychotherapy notes,

(5) To a coroner or medical examiner; or

(6) To prevent or lessen a serious and imminent threat to the health or safety of a person or the general public.

2. Minors. We will follow West Virginia State law when using or sharing PHI of minors. Minors who receive health care services related to HIV/AIDS; STDs, mental health treatment, alcohol/drug testing, and treatment or reproductive health may request that another person receive that information on their behalf. If the minor does not give permission in writing to anyone, we will only release that information to the minor.

3. Marketing Communications: Sale of PHI. We must have your written permission before using

or sharing PHI for marketing or the sale of PHI, consistent with the related definitions and exceptions set forth in HIPAA.

Other Uses and Releases. Any requests for information besides those described in this Notice will need your written permission. For example, you will need to sign a permission form before we can send PHI to your life insurance company or to your attorney. You may revoke your permission at any time by providing us with written request.

II. YOUR INDIVIDUAL RIGHTS

A. Right to Inspect and Copy. You may request to see your medical records billing records in order to inspect and/or request copies of the records. All requests to view records must be made in writing. Under limited circumstances, we may deny access to your records. We may charge a fee for the cost of copying and sending records you request.

B. Right to Alternative Communications. You may request in writing to receive PHI by alternative means of communication or at alternative locations.

C. Right to Request Restrictions. You have the right to limit PHI we use or share for treatment, payment, or health care operations. You must request limitations in writing addressed to Vicki Johnson, HIPAA Privacy Officer. We are not required to agree to limitations you request, unless your request is to limit releasing PHI to a health plan for payment or health care operations and that PHI directly relates to a health care item or service that you or another person or entity on your behalf paid in full.

D. Right to Accounting of Releases. You may request in writing an accounting of releases of PHI made by us in the last six years, subject to certain restrictions and limitations.

E. Right to Request Amendment: You have the right to request that we amend your PHI. Your request must be in writing, and it must explain why the information should be amended. We may deny your request under certain circumstances.

F. Right to Obtain Notice. You have the right to obtain a paper copy of this Notice by submitting a request to Vicki Johnson, HIPAA Privacy Officer, at (304)236-5902 ext. 104 at any time

G. Right to Receive Notification of a Breach. We are required to notify you if we discover a breach of your unsecured PHI, according to requirements under federal law.

H. Questions and Complaints. III. EFFECTIVE DATE

The original Williamson Memorial Hospital, named after Wallace J. Williamson, founder of the city of Williamson, was established in the heart of Mingo County, West Virginia in 1918. The original hospital was a three story architectural showcase.  A nursing school was established in 1920 alongside.

In 1927, a devastating fire destroyed the hospital.  Miraculously, all 33 patients escaped with their lives. Fourteen of the patients were transferred to hospitals in Huntington while others were taken to their homes.

A new Williamson Memorial Hospital was constructed in March of 1928 at a cost of $175,000.  The new, four-story facility included 72 beds, 32 private patient rooms and seven wards.  The facility featured two operating suites, a maternity ward and a nursery amongst the other services provided by hospital personnel.

Growth was a focus of hospital administrators and necessary with the growing population of the time.  An emergency room, radiology department, cafeteria, laboratories, pharmacy, an Intensive Care Unit and more medical and surgical areas were added.

In 1979, the hospital was sold to Hospital Management Associates (HMA). 8 A new medical center was completed in 1988 8 and the c. 1928 hospital building was repurposed as a medical office and clinic complex, and since 2014, as storage. 

After a failed attempt to sell the hospital to Tug Valley ARH Regional Medical Center of South Williamson, Kentucky in 2012, HMA declared the hospital insolvent.  Another failed attempt to sell to Collaborative Healthcare Solutions of New Jersey in 2018, Mingo Health Partners purchased the hospital and began the revival process.

Unfortunately, in early 2020, the global COVID-19 pandemic sent devastating effects throughout the region.  Due to financial constraints brought about by the pandemic, the hospital was forced to close its doors in April of 2020.

Shortly after its closure, Dr. Donovan “Dino” Beckett, who trained at the hospital as a medical student and worked at the facility for more than 15 years, purchased the hospital’s assets with the goal of reopening the hospital. 

Today, Williamson Memorial Hospital is a regional hub for high-quality healthcare and wellness.  We continue to work hard to improve the health and wellness in our community.
